This program is designed for participants living with a psychosocial disability in the community i.e. in a SIL house, SDA-SIL, their own home or hospital leave. The support professionals in this program hold a mental health Cert IV as a minimum, they also undergo training which focuses on observations and conversational skills, which enable them to identify potential relapses. Medication management particulary schedule controlled medications and how to report effectively on their efficacy. They are also trained in the key aspects of collaborating with mainstream health services. This model has been a success in transitioning participants who have been in long term hospital for up to 15 years, they now live meaningful productive lives in the community due to the supports provided in this model of care.
